History
Reunion Plaza Healthcare & Rehabilitation Center is in Tyler, Texas, the largest city in East Texas. From its humble beginnings in the mid-1800s, Tyler now serves as the healthcare center of East Texas, with four hospitals, including a level 1 trauma center. Tyler is known as the “Rose Capital of America,” and in the spring our town is blanketed in the vivid colors produced by blooming azaleas, dogwoods and spring flowers.
We’re part of the Southwest LTC community. Southwest LTC manages and operates several skilled nursing, rehabilitation, and assisted living facilities across Texas and Oklahoma. We provide innovative leadership in all aspects of skilled nursing and rehabilitative care.
